Purpose of the examination

A brain scintigraphy makes it possible to examine the activity of particular nerve connections (known as dopamine transporters). This exam may be indicated for the following issues, among others:

  • Differentiation of parkinsonism (for example, Parkinson’s disease, Multiple system atrophy, Progressive supranuclear palsy) from essential tremor

This examination is covered by social health insurance. The radiopharmaceutical is ordered for each patient individually (cost: about €900) and it cannot be stored for long periods of time. For this reason, keeping your appointment is absolutely essential.

Allow about 4½ hours for the procedure, which includes a 3-hour break.

Diagnostic procedure

One of our doctors will take plenty of time to fully discuss your symptoms, any previously performed tests and treatments, as well as the procedure of the planned examination.

In order to protect your thyroid gland from unnecessary stress, you will first receive 40 drops of Irenat®. To begin, a trace of a radioactive substance is injected into your arm. There has never been any report of an allergic reaction to the substance we use. The radiation exposure is lower than that of a CT scan. The substance will accumulate in the central brain, known as the stem ganglia, within 3 hours.

While the medicine is taking effect, you are free to leave the clinic. You are free to eat or drink.

The scan will be made with you lying down. Then, a large camera (the “gamma camera”) slowly rotates around your head for 45 minutes. This machine is not a closed tunnel. It is open to each side, and makes no noise. Therefore, patients suffering from claustrophobia tolerate the process well. Throughout the examination, one of our medical technicians will assist you and remain within hearing distance.

Evaluating the scan takes quite some time, as it requires complicated calculations. The resulting images will be printed out and given to the consulting doctor.

The written report is prepared on the same day or at the latest on the following day and is sent to the referring physician and the patient.

Medications containing Bupropion, Methylphenidat and Sertralin must be discontinued before the examination. Please phone us to inquire when to discontinue your medication.

Please bring a list of your current medications.

If you had an MRI of your head, please bring the images and/or the results.

Untersuchungsablauf

[1] Vorgespräch
In einem ausführlichen Gespräch besprechen Sie mit einem unserer Ärzte Ihre Beschwerden, die bisher durchgeführten Untersuchungen und Behandlungen, sowie den Ablauf der geplanten Untersuchung.

[2] Injektion
Um die Schilddrüse vor einer unnötigen Belastung zu schützen, erhalten Sie gleich zu Beginn 40 Tropfen Irenat®. Im Anschluss wird eine Spur einer radioaktiven Substanz in Ihre Armvene injiziert. Allergische Reaktionen gegen die verwendete Substanz sind nicht bekannt. Die Strahlenbelastung ist geringer als bei einer Computertomographie. Die Substanz reichert sich im Laufe von 3 Stunden in den zentralen Hirngebieten, den sog. Stammganglien an.

[3] Einwirkzeit
Während dieser Einwirkzeit können Sie die Praxis verlassen. Sie können auch gerne etwas essen oder trinken.

[4] Scan
Die Aufnahmen werden im Liegen durchgeführt. Sie erfolgen mit einer großen Kamera, der sog. Gammakamera, die in ca. 45 Minuten langsam um Ihren Kopf rotiert. Bei dem Gerät handelt es sich um keine geschlossene Röhre. Es ist zu den Seiten hin offen und macht keinen Lärm. Daher werden die Aufnahmen auch bei bestehender Platzangst meistens gut toleriert. Während der gesamten Untersuchung werden Sie durch eine unserer MTAs betreut, die sich ständig in Rufweite befinden.

[5] Archivierung/Druck des Untersuchungsergebnis
Die Auswertung der Aufnahmen nimmt einige Zeit in Anspruch, da ein aufwendiger Rechenprozess notwendig ist. Die erstellten Bilder werden auf Papier ausgedruckt und dem behandelnden Arzt zur Befundung vorgelegt.

[6] Nachbesprechung
In einem abschließenden Gespräch bespricht der Sie betreuende Arzt mit Ihnen den Befund. Den Arztbrief, die erstellten Aufnahmen sowie die CD-ROM bekommen Sie in der Regel gleich mit.

Wichtige Hinweise

  1. Das Radiopharmakon wird Patienten bezogen (Kosten ca. 900€) bestellt und ist nicht lagerfähig. Deshalb ist die Termineinhaltung unbedingt erforderlich.

  2. Medikamente mit den Wirkstoffen Bupropion, Methylphenidat und Sertralin müssen vor der Untersuchung abgesetzt werden. Bitte rufen Sie uns bezüglich der Absetzungsdauer an.

  3. Bitte bringen Sie eine Liste Ihrer Medikamente mit.

  4. Falls bereits eine Kernspintomographie (MRT) des Kopfes erfolgte, so bringen Sie die Bilder, bzw. den Befund bitte mit.
