With Ga-68-HA-DOTATATE PET/CT, images can show the presence on cell surface structures (so-called somatostatin receptors) of specific tumours of the gastrointestinal tract (neuroendocrine tumours, so-called NET). Thus, the primary tumour and possible metastases can be detected and evaluation is also possible for treatment with octreotide (so-called sandostatin), or radiolabelled octreotide (Y-90 or Lu-177-DOTATATE, so-called peptide receptor radionuclide therapy, as part of the “theranostic” concept). Here on the one hand, PET (positron emission tomography) is used to image tumour metabolism, on the other hand, the CT (computed tomography, here as “low-dose CT”) is used for spatial localisation. The combination of both methods in one examination allows the exact localisation of malignant tumours in a way that it is not possible with other methods. The fees for PET-CT in Germany are not currently covered by the statutory health insurance (GKV) for this indication. We will gladly assist you in creating a cost-transfer application.

The examination may also be indicated for other problems, including the following:

  • Diagnostic localisation for NET tumours (including carcinoid, gastrinoma, insulinoma)
  • Problems involving  recurrent tumour growth after treatment (so-called relapse), metastases.
  • Selection and treatment control with respect to peptide receptor radionuclide treatment 
  • C-cell carcinoma of the thyroid, Merkel Cell Tumour

On the day of the examination, it is not necessary to abstain from food and drinks. Medications containing the active ingredient “octreotide” (Bendatreotid, Sandostatin, Octreotid) must be discontinued 24 hours before the examination. Please bring along to this examination any relevant previous findings. Allow a good 2-3 hours for the examination.

Diagnostic procedure

In a detailed interview you will discuss with one of our doctors your symptoms, examinations conducted previously, along with treatments, as well as the procedures used in the scheduled examination.

Following this, an intravenous injection will be made into a vein in your arm containing a trace of a radioactively labelled substance that attaches to the somatostatin receptors of the tumour tissue. Therefore, an exposure time of about 1 hour is necessary before the images can be created. The substance used is very well tolerated. Allergic reactions are not documented. The radiation exposure is about 4-6 mSv, about the same as a diagnostic CT of the spine.

After that, you can relax in our waiting room. You should drink at least 1 litre of mineral water and frequently empty your bladder. This improves the quality of the imaging. Mineral water is readily available at our office.

The imaging is performed while lying down and will take about 45 min. It is done with a so-called PET scanner. This device has no closed tube, but two separate ring systems which are arranged one behind the other with some spacing. In between these the unit is open. Therefore, the imaging is usually well tolerated even for people who are claustrophobic. Throughout the investigation you will be looked after by one of our medical technicians, who will be in permanent earshot.

The results of the examination are evaluated by the attending physician on a monitor, individual images are printed out on paper and the entire record of the investigation is archived onto a CD-ROM.

The written report is prepared on the same day or at the latest on the following day and is sent to the referring physician and the patient.

Untersuchungsablauf

[1] Vorgespräch
In einem ausführlichen Gespräch besprechen Sie mit einem unserer Ärzte Ihre Beschwerden, die bisher durchgeführten Untersuchungen und Behandlungen, sowie den Ablauf der geplanten Untersuchung.

[2] Injektion
Im Anschluss wird eine Spur radioaktiv markierter Substanz in Ihre Armvene injiziert, die sich in stoffwechselaktivem Prostatagewebe/-metastasen anreichert. Die verwendete Substanz wird sehr gut vertragen, allergische Reaktionen sind nicht beschrieben. Die Strahlenbelastung ist vergleichbar mit einer diagnostischen Computertomographie der Wirbelsäule.

[3] Wasseraufnahme
Sie verweilen dann bitte in unserem Wartezimmer. Sie sollten 1/2 Liter Mineralwasser trinken und häufig die Harnblase entleeren. Dadurch wird die Aufnahmequalität verbessert. Mineralwasser ist in der Praxis ausreichend vorhanden.

[4] Scan
Die Aufnahmen werden im Liegen durchgeführt und dauern ca. 45 min. Sie erfolgen mit dem sog. PET-Scanner. Bei dem Gerät handelt es sich um keine geschlossene Röhre, sondern um zwei einzelne Ringsysteme, die mit etwas Abstand hintereinander angeordnet sind. Dazwischen ist das Gerät offen. Daher werden die Aufnahmen auch bei bestehender Platzangst meistens gut toleriert. Während der gesamten Untersuchung werden Sie durch eine unserer MTAs betreut, die sich ständig in Rufweite befinden.

[5] Archivierung/Druck des Untersuchungsergebnis
Die Untersuchungsergebnisse werden von dem behandelnden Arzt am Monitor befundet, einzelne Bilder werden auf Papier ausgedruckt und der gesamte Datensatz der Untersuchung auf CD-ROM archiviert.

[6] Nachbesprechung
In einem abschließenden Gespräch bespricht der Sie betreuende Arzt mit Ihnen den Befund. Den Arztbrief, die erstellten Aufnahmen sowie die CD-ROM bekommen Sie in der Regel gleich mit.

Wichtige Hinweise

  1. Sie müssen am Untersuchungstag nicht nüchtern sein.

  2. Medikamente mit dem Wirkstoff “Octreotid” (Bendatreotid, Sandostatin, Octreotid) müssen 24 Stunden vor der Untersuchung abgesetzt werden.

  3. Das Radiopharmakon wird Patienten bezogen hergestellt und ist nicht lagerfähig, deshalb ist die Termineinhaltung unbedingt erforderlich.
